fixation

Patella sleeve fractures

Ossification

3 - 5 years old

Management

Undisplaced

- manage in plaster in extension

Displaced > 3 mm

- ORIF

Patella sleeve fracture

- usually small inferior fragment seen on xray

- patella high riding on xray / alta

- is actually large cartilaginous fragment avulsed with retinaculum

- can be osteochondral

Conformity Materials Fixation Gender

Conformity

Definition

Geometric measure of the closeness of the fit of the knee articulation

Concepts

The ideal TKA maximises articular conformity while minimising axial constraint

Highly conforming / Fully congruent

- constant sagittal femoral radius

- large contact areas

- theoretical limitation of flexion to 120o due to posterior impingement of the tibial component

Latarjet / Bristow

Bristow

Concept

Non-anatomical bony block

- transfer of coracoid process through subscapularis

- dynamic anteroinferior musculotendinous sling

- provides subscapularis tenodesis

- preventing lower portion from displacing proximally as arm abducted

- when shoulder in vulnerable position abduction and ER

Capitellar OCD

Epidemiology

Adolescents & young adults

Usually between 12 - 21 years

Throwing athletes / gymnasts

Little Leaguer's Elbow

- combination of capitellar OCD and MCL injury

- a repetitive throwing injury / seen in pitchers

Aetiology

1.  Trauma & Overuse

Common throwing sports / gymnastics
