Enchondroma

Epidemiology

 

Most common bony tumour of hand

 

Risk malignant transformation isolated lesion is < 2%

- more likely in long bones than hands

 

Site

 

Proximal phalanx > middle

 

Syndromes

 

Ollier's

- multiple enchondromatosis

- risk of transformation 10 - 25%

 

Maffuci's

- enchondromas and hemangiomas

- risk transformation close to 100%

 

Prognosis

 

Observation unless

- pain

- aggressive X-ray changes

- increased uptake on bone scan

 

Biopsy to exclude malignancy

 

Pathological fracture

 

Allow fracture to heal

- enchondroma does not resolve

 

Curette and bone graft at a later date
